The GPs at CHC are part of of NHS Hounslow Clinical Commissioning Group   (CCG). This is the GP-led organisation responsible for planning and buying   (commissioning) health services for the people living in Hounslow. The group  is made up of 54 GP Practices serving a registered patient population of  288,000. They are committed to improving the care provided to patients, reducing health inequalities and raising the quality and standards of GP  practices whilst achieving a financial balance. There is an excellent web site  for HCCG which has lots of information and is worth while visiting. Click HERE to visit it.  The CCG is overseen by NHS England which makes sure they have the  capacity and capability to commission services successfully and to meet the  financial responsibilities of the CCG.

At Chiswick Health Centre there are three GP Practices and also other
services which come under the aegis of Hounslow and Richmond Community  Healthcare NHS Trust. They are the NHS trust responsible for  providing community health services for over 500,000 people living in the  London boroughs of Hounslow and Richmond upon Thames.  They provide a wide range of health services for adults and children  including community nursing, health visiting, physiotherapy, smoking  cessation, neuro-rehabilitation, speech and language therapy, occupational  therapy, and health and well-being services.

The GPs use an external service called the Referral Facilitation Service when  they refer patients to hospital. If you have been referred to a hospital or clinic through the NHS e-Referral Service, you can book your appointment online. Click HERE to go to the online booking service
